A new study has found that a medication known as finasteride may have an increased risk of heart attack in people who already have a heart condition. Dr. Richard L. Rosenblatt, a urologist at the Cleveland Clinic, who was not involved in the study, said the findings are "significant" because it could lead to more people being diagnosed with heart disease, especially in those with a history of heart disease.
Finasteride, known generically as 5-alpha-reductase inhibitor, was originally developed as a treatment for benign prostatic hyperplasia (BPH). However, this drug wasn't fully approved by the U. S. Food and Drug Administration (FDA) until 1997, when Merck & Co. began selling it as a treatment for enlarged prostates. A few years later, the FDA approved Proscar for BPH.
Finasteride was originally developed to treat male pattern baldness, a condition characterized by an enlarged prostate. Proscar was initially approved in 1997 for this condition, but the FDA did not approve it until 1999 when Merck & Co. sold Proscar for male pattern baldness.
About 5 percent of men between the ages of 18 and 41 suffer from BPH, which is a condition characterized by an enlarged prostate. When treating BPH, finasteride works by decreasing the amount of an enzyme called 5-alpha-reductase, which converts testosterone to dihydrotestosterone (DHT). DHT is the hormone that causes the prostate to grow larger. Proscar is a 5-alpha-reductase inhibitor, which prevents the conversion of testosterone into DHT.
It's important to note that the risk of prostate cancer is increased in people with high DHT levels. Men who take finasteride may also be at an increased risk of developing breast or uterine cancer, which can be caused by an excessive amount of testosterone in the body, including DHT.
Finasteride may also increase the risk of developing diabetes, an condition characterized by low blood sugar, high cholesterol and diabetes-related blood sugar levels. In addition, finasteride may increase the risk of developing a type of prostate cancer.
The study was supported by grants from the National Institutes of Health to M. I. and D. and by the National Heart, Lung, and Blood Institute of the National Institute of Diabetes and Digestive and Kidney Diseases to M. The other authors declared no conflicts of interest.
